Be Sure to Renew Your Medicaid Benefits Annually!
DID YOU KNOW? You can lose your health care coverage if you do not take action to renew every year.
The state of Ohio sends a reminder form in the mail. When you receive it, or when you receive a reminder from your managed care provider (e.g., Buckeye, CareSource, Molina, Paramount, UnitedHealthcare), it’s time to take action. Always confirm or report a change in address to Job & Family Services (JFS) office so you won’t miss your renewal reminder.
WHAT DO I NEED TO DO?
RENEW YOUR COVERAGE IN ONE OF THESE WAYS:
